Our Policy
PHCA complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, sex, gender identity, or sexual orientation in the provision of medical care or in our hiring practices. We provide the same quality of care to every patient who comes through our doors.
Language Access
PHCA provides free language assistance services to patients and families with limited English proficiency, including qualified interpreters and information written in other languages, upon request. Please let our front desk know if you need language assistance for a visit.
Accessibility
Our Altoona and Roaring Spring offices are wheelchair accessible. If you or your child need an auxiliary aid or service to communicate effectively with us — such as sign language interpreters or written materials in alternate formats — we will provide these free of charge.
Grievance Process
If you believe PHCA has failed to provide these services or discriminated in any way, you can file a grievance with our office:
- In person, by phone, or in writing to our Practice Manager at either office location
- By phone at (814) 944-7383
- By email at phca5@yahoo.com
We will investigate every grievance and respond within 30 days. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically through the Office for Civil Rights Complaint Portal, or by mail or phone.
No Retaliation
PHCA will never retaliate against a patient or family for filing a grievance or raising a concern about discrimination. Doing so in good faith will never affect the quality of care your child receives from us.
